Our Services:
HomeQuest provides Housing Stabilization Services. This is a new Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and keep housing.
How does someone qualify for HomeQuest services?
To be eligible for Housing Stabilization Services, members must meet the following criteria based on their needs:
Must be on Medical Assistance (MA)
Must be 18 years or older
Must have a documented disability or disabling condition, which is defined as one of the following:
A person who is aged, blind, or has a disability as outlined under Title II of the Social Security Act.
A person with an injury or illness expected to cause extended or long-term incapacitation.
A person with a developmental disability (or related condition) or mental illness.
A person with a mental health condition, substance use disorder, or physical injury who required a residential level of care and is now transitioning to the community.
A person determined to have a learning disability as defined by the Minnesota Department of Human Services (DHS).
A person with a substance use disorder currently enrolled in a treatment program or on a waiting list for a program.
Must require assistance with at least one of the following areas due to a disability or long-term condition:
Communication
Mobility
Decision-making
Managing challenging behaviors
Must be experiencing housing instability, as evidenced by one of the following risk factors:
Homelessness: The individual or family lacks a fixed, adequate nighttime residence.
Transitioning from an institution or licensed/registered setting: This includes settings such as a hospital, nursing facility, or adult foster care.
At risk of homelessness: The individual or family is at risk of becoming homeless due to circumstances such as living in a condemned building, having arrears in rent or utilities, receiving an eviction notice without a place to move, or living in temporary housing with time limits.
Discharged from an institution: The person, previously homeless, will be discharged from a correctional, medical, mental health, or substance use treatment facility and lacks the resources to secure housing.
At risk of institutionalization: The individual meets an institutional level of care or is eligible for the following waivers:
Brain Injury (BI)
Community Access for Disability Inclusion (CADI)
Community Alternative Care (CAC)
Developmental Disability (DD)
Elderly Waiver (EW)
